Every project must complete and upload Forms 1, 1A, a Research Plan, and 1B.

Projects with additional risks, such as human subjects, vertebrate animals, and potentially hazardous materials like chemicals and animal tissue, require additional forms – as do continuation projects. All forms must be completed prior to experimentation, with the exception of Form 1C.
